How to use the checklist: 1. The auditor should enter the functional areas and their numbers pertaining to the institution under audit into the Audit form 1. It will be ideal if the quantity and type of waste are separately specified. This will help to benchmark and monitor the performance of the institution. 2. Sufficient copies of the Audit form 2 should be printed and the activity in each functional area described on a scale of 1 to 5 (1 is excellent and 5 is very poor). In case of more than one unit (rooms) in any of the functional area, a simple arithmetic average could be calculated by dividing the sum of individual scores by the number of units. |

| 3. The result of the Audit form 2 should be transferred to Audit form 3, which will directly compute the ranking of each functional area and also the ranking for each activity separately. | |||
